Anyone ever see anything like this, or know what "might" cause the break and distortion?

The only thing I can think of is maybe someone took something and put a lot of pressure on that wheel to try and make the watch run or run faster..screwdriver maybe..

Replaced the complete wheel/pinion - and running fine.

The pinions were not broke

There does look to be "scratch" on the wheel itself three (3) teeth up from the break might have been a screwdriver impact like Gary alluded to. Crazy...

Unmitigated abuse by ice-pick (or screwdriver). Those three breaks are all fractures.

I favor the ball point pen theory. Stick the pen point into the watch to "help the little wheel thing move better".
